Telovelar Approach for Resection of Giant Pontomesencephalic Cavernous Malformation

Cavern malformation surgery 48 mm

This video is about the surgery of a cavernous appendage or cavernous malformation in the medulla, which took a large part of the patient's medulla, in fact, it was one of the largest appendages discussed in the literature of brain surgery, because its dimensions are about 48 mm, which is approximately 5 cm. For this part of the brain, where the medulla is between 5.5 and 6 cm, it occupied almost 90% of the space in that part.

The patient is a 50-year-old person who came with weakness on the right side and weakness of eye movements on the left side, and his MRI showed 9 cave abnormalities in the brain, one of which was related to the patient's movement problems and weakness.

Cavernous Malformation disease is often silent and does not show any symptoms, but sometimes it grows and bleeds. Due to the problems created for this patient, the surgical team decided to operate on him and removed a part of the appendage with a craniotomy of the base of the posterior skull, and at the same time carefully removed a part of the vein, which should not be damaged or removed due to its influence on the circulation of blood in the medulla. They kept it.

In the stage three surgery that was performed after a few weeks, the bleeding of the previous stage was removed, and the entire appendage was removed without putting pressure on the brain.

The advantage of surgical staging is that after some time, a layer is created between the brain and the site of the appendage, which makes removing the appendage easier and less likely to harm the patient. Therefore, this method is recommended in cases where the appendage is very large.
